File upload and deletion

Hi everyone, I'm trying to write a form that hides email addresses from
spiders and allows for a single file upload. Here is a link to what I have

I'm having
trouble with the file upload.
What should happen is:
-The user should be able to attach or not attach a file.
-If a file is uploaded it is mailed then deleted.
-If a file is not attached then the attachment section is ignored and
everything else is mailed.
I'm still very new to PHP, about a 3 weeks in now. Any help would be great.
