Try compiling PDFlib with:
--enable-shared-pdflib
I believe there is also a patched version of pdf.c that needs to be placed
in the PHP source. I have heard people say that PDFlib 3.01 and 3.02 won't
compile with 4.0.2 but not sure about 4.0.1pl2.
