I would do it something like the following (semi psuedo code - i'm lazy)
Have an errordocument in .htaccess which is a php file
The php error handler grabs the url that was trying to get to.
Breaks it down into basename, basepath (eg /pics/index.htm)
basename = index.htm
basepath = /pics
Check in the basepath directory for something matching basename - php has
similar text functions for this.
Make a best guesstimate (probably an array of possible files, returned from
a function?)
If guesstimates found, show the link(s) to that.
If no files match in the basepath, or the basepath is wrong, do a a similar
name text search from the / to suggest similar directories / files.
I've been thinking about writing one of these for a while, just been busy...

This is not a PHP question!
But, there is an option in Apache confs to try and guess from the
bad hit what they were looking for. Not sure what it is or
how it works, but it is possible!
