Input form strategy?

I would like to get some opinions or references on the design of an
Input Form.
Currently, I send a flag via the URL, "redo".

$redo = $HTTP_GET_VARS[redo];
if ($redo) {
$firstname = $HTTP_GET_VARS[firstname];
$lastname = $HTTP_GET_VARS[lastname];
} else {
$firstname = "";
$lastname = "";
};
//Put the INPUT fields here and set values from the above variables

I would like to tell what is wrong with the form on redo's. This is
something I am not sure about - how to send in messages e.g. "The first
name must be filled in".
