PHPLIB has classes which you can use to create queries, then few classes to
create forms etc...
it might be some kind of hard to understand, but it was written very, very
well...
otherwise just go to a script libraries like hotscripts.com, look for
databases, get some with search feature, then copy the (non-perfect) code
and modify it as you wish... (bad solution, but if you think you would do it
anyway, then here's how...)
I recommend you to write your own, it's not too hard, just a form then a
script to generate a smart SQL query..
