Many can be found at phpbuilder.com

I like my forms all in an array -
ie <input type="text" name="form[firstname]"> then
$form['firstname'] will hold
the value of whatever the user put in.

header("Location: http://www.server.com/second_page.html?firstname=".$form['firstname']);
