The latter I think, but cachíng proxies *do* pay attention.

Yes, but I am generating *the same page* as before quite often. I
would like a way to tell PHP and APACHE that this is the case, so it
could stop processing the rest of the file (or better, not process
it at all) and just tell the cache it hasn't changed.

You mean, go back to writing pages with CGI and SHELL scripts? No
thanks. Even if PHP adds a new twist on that, it's still doing a
much better job than otherwise.
But if we are serious about allowing PHP pages to get obscene
amounts of hits, we need a method of addressing conditional
requests. It makes a *huge* difference in the speeds of pages, and
allows better serving structures.
Both are big $$$ items. In addition, I like my pages to be the best
they can be. :)
