Apache segfaulting

Hi all,
I'm having a huge problem with php4 causing apache to segfault.
I have a file upload section on a site and the problem is that the files
uploaded are assigned an id code which is stored in a postgreSQL
database. However, I now sometimes have ghost files or users with
no files.
I include the snippets of code for the upload form as well as the php4
catching it. Please tell me what I'm doing wrong as I am now clueless
