Trying To Figure out A Loop

Hello everyone, I am trying to loop through a mysql database and output this information in html tables. I have done something like this once before in a script, and I think I am following the same strategy as before. Right now however, although there are three rows in the table, it is only outputting one row of information. In the code, include ('categoryhtml.html'); this is the html with the php in it that should be outputted.
