Need help

Hello-- I need help with some mail forms I am implementing on my webpage. I have set up a form where users will type in text and select various options from pull down menus. I want to put in a text field where the user can specify an e-mail address where they would like to send their responses to. How do I go about doing this or where can I look at sample code that does this?
