problem with form

This form:
<FORM ENCTYPE="multipart/form-data" ACTION="trashthis2.php3?mode=save"
METHOD=POST>
<input type="text" name="name" size="40">
<input type="text" name="city" size="20">
<input type="text" name="state" size="15">
<input type="file" name="userfile" size="25">
<input type="submit" value="Send Form">
</form>
Results in the following error:
ERROR
The requested URL could not be retrieved
While trying to retrieve the URL: [no URL]
The following error was encountered:
Zero Sized Reply
Squid did not receive any data for this request.
This form works correctly:
<FORM ENCTYPE="multipart/form-data" ACTION="trashthis2.php3?mode=save"
METHOD=POST>
<input type="file" name="userfile" size="25">
<input type="text" name="name" size="40">
<input type="text" name="city" size="20">
<input type="text" name="state" size="15">
<input type="submit" value="Send Form">
</form>
The only difference is that the <input type=file"> has been moved to the
top of the form.
Both of these forms work correctly on other web servers, but the first
form has started producing errors on one of our servers. This is something
that has just popped up over the weekend I believe.
I suppose if I have to, I go through all the forms like this on this one
server, and move the offending input tag to the top. But one that would be
a pain, and two, if I have to, I sure would like to know why???
Any help of suggestions would sure be appreciated.
