Yes, but not with a single INSERT statement.
You can put as many queries as you want in the form action script, so
that they all execute upon a single form submission.

No. There's no connection between form data and SQL queries, really. You
could have a single html form with 5 fields, each of which go to 5
different SQL tables, if you wanted.
