Have a look at the very nice PHP windows binary distro at www.php4win.de as
it has a working PDFlib modules (plus a stack of others)

Subject: PDFLib in NT/IIS
Could someone explain the process of getting this to work for me ? So far, I
have enabled the extension to the php_pdflib.dll. But once I run a script
with pdf functions,it tells me that it can find 'pdflib.upr'. I've tried
placing this file and it's fonts in important directories,\system32, the php
isapi dll directory, and also the directory where the script is running
from.
Is there anyway someone could show me how to get this to work on NT ?
