file upload path on WinNT

We're trying to get a file uploaded to a server running PHP4 on WinNT.
We have the form set up correctly (as far as I can tell) for an image (which
we called smallimg).
When we submit to the script, the temp path ($smallimg) always comes up as
"none" which makes it impossible to successfully copy the file (couldn't
open "none" for reading...)
Any ideas as to what we are doing wrong?
