Use JavaScript to create a form like this:

I am using this to pass a value to a php page that then generates property
tax info for a town. In my example above "PROPERTY TAXES" looks like a
hyperlink but it is actually the submit button for the form. I have 5
different values (officials, census data, etc.) that I place into separate
JavaScript forms in a HTML table.
