Because browsers don't follow spec.
IE ignores Content-Disposition. Or maybe it's Netscape. And only certain
versions... Either way, that problem of fake filenames and
Content-Disposition is separate from not wanting to use ? and &.
Even if you just have http://yourserver.php which returns a GIF
or whatever,
and the user tries to right-click or otherwise download it, they'll get
messed up if you rely solely on Content-Disposition.
The reason to use / instead of using ? and & is two-fold. One, to get
search engines to index it, and two to make the URLs slightly more
user-friendly to humans to type. It *still* won't fix the broken browsers
that ignore Content-Disposition unless you tack on a something.gif for a GIF
etc.
