In the Apache httpd.conf file, assuming you have PHP 4 configured as a module or CGI version properly, so Apache knows what to do with application/x-httpd-php (note: no number after the php for php4), then you just need the lines below (first for .php3 files, then .phtml, etc).
AddType application/x-httpd-php .php3
AddType application/x-httpd-php .phtml
AddType application/x-httpd-php .php
AddType application/x-httpd-php .php4
