Look at:
http://cvs.php.net/viewcvs.cgi/php4/win32/install.txt
and click on the first download link. Follow the directions for the Apache module version, but I would recommend adding:
AddType application/x-httpd-php .php
AddType application/x-httpd-php .php3
AddType application/x-httpd-php .phtml
in addition to:
AddType application/x-httpd-php .php4
so it will process php, php3, and phtml files as well.
Also, if you installed the CGI version into the Apache configuration before, be sure to remove those lines.
