application of Jay Bloodworth's FORM CLASS?

Dear list, I found Jay Bloodworth's Form class (1.1) on a web site. The class comes with a short explanation but the way to build a page that tests itself is not very clear. To me. I know figured how to build the form and i even get the javascript validator, though i don;t have a good regexp yet. I am looking for real life examples of forms using that class, complete with inclusions, and server-side validation. please?
