One issue ive seen with stuff like that is if you have a picture of a bunny some people will call it
a rabbit, or a buny, etc etc
Especially bad with non-native speakers who may not know the word for something

Such visual-only CAPTCHAs might be impossible to solve by visually impaired people. It's
better to provide a fallback for them, what might be even required by law.
