Make sure that the <? that starts the php script is the VERY first line of the file, and that there are NO spaces before it anywhere. I had the same problem, I was used to html and even PHP being rather syntax flexible. As soon as I removed the spaces and extra line at the beginning of the file, life was back to normal. Let me know if this works.
