You'll have to separate it of course. You have one script which fetches the
image data, this is to be treated exactly in the same manner as an image.
And one that generates your page, or actually it can be a pure html-page if
necessary. =)

To the public:
I've seen alot of similiar questions in this mailinglist, and it's
siblings.. frankly I'm getting a bit tired of downloading hundreds of mails
each day with mostly generic questions. I'd take the time myself to compile
a Q&A on common subjects if I had any to spare. =)
And I don't really think that most new users take the time to browse
through a lot of resources to find this sort of information, it is often
easier to ask for it in a mailinglist and wait for a reply. If a Q&A on
this is to be compiled, it should be put up as a document on the official
PHP site.
I'm probably not the only one who has thought of this, but still =)
