This is claiming PHP is running as a CGI. PHP running as a CGI cannot do
authentication headers. (I think because the data would be passing
through insecurely from server to CGI, so it ain't never gonna happen.)

Are you 100% certain you are compiled as NSAPI Module? What does phpinfo() say?
Actually, I don't know enough about NSAPI to know if it is capable of
authenctication headers or not, so maybe you are using NSAPI Module, but
whatever makes CGI not do authentication is limiting you as well...
