Are you sure the user who is trying to create the user has permissions to all the necessary files ( /etc/{passwd,group,shadow} ) etc. ? Given that it appears your on a linux box, are you using a kernel version that supposed set{u,g}id apps? few issues.. some more details as to how you are trying to use PHP would be helpful..
