try the query like this: mysql_query("INSERT INTO members (email, uname, fname, lname, city, state, phone, address)VALUES('$email','$uname','$fname','$lname','$city','$state',' $phone','$address'") or DIE("ERROR : ".mysql_errno() ." <b>".mysql_error()."</b>"); But, honestly I think you're missing few spaces to surround ')VALUE(', hey, it thinks it's a funciton or something.
