I use php as a CGI from a secure location to do it all.
To add a user to .htpasswd on Apache:
$thefile=fopen("$pathtofile/.htpasswd", "a");
$newuserline="$username:" . crypt($pass) . "\n";
fwrite($thefile, $newuserline);
