If I know well, "Undefined index" is a warning, not an error.
If you have
error_reporting = E_ALL
in php.ini (not error_reporting = E_ALL & ~E_NOTICE)
$a = array('a' => 'xxx', 'b' => 'yyy');
and you try to access $a['c'], then PHP will report that you are trying
to access an undefined index array.
