You cannot send ANY text or html tag (even a space ) before usin header
function.
If you have to get something displayed.
I suggest to use then :
echo "<HEAD><META HTTP-EQUIV=\"Refresh\" CONTENT=\"4;
URL=login.php?failure=true\"></HEAD>";
