This message is looking more like an otf_GIF message than a PHP one to me,
the village idiot... Is it?
Does otf_GIF work under ASP as a COM object?... I assume so, but have to ask.
CoInitialize looks like it's maybe somehow related to threads or
concurrent processes...
Are you running PHP as a CGI or does O'Reilly let you use it as a Module?
Is it possible that the code is not re-entrant, but as a Module, PHP is
expecting it to be?
See if there's any sort of time-dependency in it: If you wait 1 minute to
reload the second time after restart, does it die or does it work again?
