It seems to me that the use of a temp variable may be the clearest solution, and I doubt any overhead would matter really.
$a = "dog";
$tmp = "MY".$a; //$tmp = "MYdog";
So the following two would be the same:
$$tmp = "Spot";
$MYdog = "Spot";
