Is
cat header body footer > output
too simple? Any reason why it has to be PHP? Suppose you could do :
<?
$header = "header.txt";
$body = "body.txt";
$footer = "footer.txt";
$output = "output.txt";
exec("cat $header $body $footer > $output");
?>
