You're concatenating "echo $PHP_SELF" rather than just $PHP_SELF, which
isn't necessarily helping. But just between me and you, life would be a
lot easier if you simply did:
echo '<form name="frmMovies" method="post" action="' .
$PHP_SELF . '">';
