Fixes #24584.
Treat OOM as fatal by default by changing the default
memory_hard_limit_percent to 100,
so exceeding memory_limit terminates the process immediately instead of attempting
recovery. The hard-limit check now also applies to fallible allocations.
The previous recoverable behavior remains available by setting
memory_hard_limit_percent=-1 (or a value above 100).
