Did you try with a fresh checkout since yesterday morning? I hadn't found the GTK_SHARED cflag until then - that ought to help you some. Plus you need to include gen_gtk.h via php_gtk.h - I didn't apply this in CVS because it's core stuff and Andrei might need it to be absent for now.
