I found you can get around the hyphen issue if you don't mind abandoning 'php-gtk' as the prefix. 'gtk' or 'phpgtk' will work just as well.
You just need to rename the INI declarations (and any reference to them) in php_gtk.c. I thought these were tied up with the module declaration in Zend/TSRM, but apparently not so:
//php.ini
error_reporting=E_ALL|E_STRICT
extension_dir=./
date.timezone=Europe/London
extension=php_gtk2.dll
phpgtk.codepage=CP1250
phpgtk.extensions=php_gtk_libglade2.dll
C:\sandbox\1053testarea>php -r "print_r(get_declared_classes());"
// lots of stuff..
[339] => Glade
[340] => GladeXML
)
php -r "phpinfo(INFO_MODULES);"
...
...

phpgtk.codepage => CP1250 => CP1250
phpgtk.extensions => php_gtk_libglade2.dll => php_gtk_libglade2.dll
...
...
C:\sandbox\1053testarea>php -r "print_r(ini_get_all('php-gtk'));"
