The last I knew there were parts of Cairo not complete in PhpGtk2; not sure if this is one or not.
Also, have you tried the PECL:
http://pecl.php.net/package/cairo
And you might want to open this conversation on the IRC, if you haven't already, since Elizabeth is there most days and she knows more about the Cairo piece than anybody else I know. Freenode: #php-gtk
