Apache doesn't send HTTP authentication to PHP - how do I set this up?

I have a PHP (4.0.4 with Apache 1.3.14 on Win98) script that uses HTTP
authentication, but $PHP_AUTH_USER and PHP_AUTH_PW are empty. Why isn't
Apache sending that information to PHP?
