I had a similar problem. I installed PHP 4.0.6 on Windows 2000 Server and IIS 5.0.
I got the same error. I checked the command line and the output looks good to me.
does the PHP.exe need to be in the cgi-bin directory? currently it is not in a web-shared
directory.

After reading all the documentation I could find, I added the permissions to all the files that I
could find that had to do with PHP: php.ini, php.exe, php4ts.dll, sessiondata, and uploadtemp for
IUSR_<SERVERNAME> (and even for Everybody). Nevertheless I always get the same error. Does
anyone have anything else that I could try? I would really appreciate any ideas anyone has.
