Can't send form by email - please help

Hi Newsgroup, I am having this problem from some time now and now I am getting quite desperate. Must admit that I am newbie to php. Tend to specialize in Cisco routers and W2K active directory. This is what I have. I am running W2K Server SP3, IIS 5.0 and installed PHP 4.2.2. Printed out Install.txt and followed it trough. Still when you click on my website: http://www.v500.com/log_a_problem.htm you won't be able to send this to me. I can send you my php.ini and script file Any help will be greatly appreciated.
