make test TESTS=ext/foo
Or, add this little shell script which I call phpt:
#!/bin/bash
TEST_PHP_EXECUTABLE=/usr/local/bin/php
php /usr/local/lib/php/build/run-tests.php $1
Then cd into whatever directory has your tests and type:
phpt .
