You submitted two account requests with the same email address. The
insert on the second one will fail because nobody is supposed to have 2
accounts. It is working exactly as it was designed. Whether or not the
email appears on internals is completely irrelevant. The people who grant
these accounts use the web interface and it appeared fine there.
