We surely may change it yes. Mainly due to better 64bit support,
size_t and int(32|64)_t usage. That will impact a lot of code, in core
or in external extensions. Help welcome in this area as it is a huge
effort.
Some remaining missing TSRMLS_* usage may be added as well.
