Hi!
ISPs are not providing PHP scripts most of the time, but they require
you to provide them one on your own. You can search for ready
made-feedback form scripts on some sites hosting packaged PHP
applications. See http://php.net/links.php#scripts
for links to some of
these sites. If you would like to program a feedback form on your own,
PHP is easy to pick up. I would suggest you read the PHP tutorial at
http://php.net/tutorial or look for some books at http://php.net/books
