HTLM to PHP

I created a multi-page (6 pages) questionnaire for my web site under MSWord 2003. The questionnaire will require the targeted visitor to fill in the form and send it to me via e-mail. I have a problem insomuch that I am required by my Host to have the action=formmail.php and method=post. Additionally, I have to modify the file formmail.php ($recipient and $referers=array). This is "latin" for me. Are you able to guide me through this task. I am a beginner (real beginner) and will highly appreciate your assistance. Thanking you in advance for your prompt reply.
