Seems like the file is there:
http://dk.php.net/source.php?url=/manual/en/index.php
Although it is not accessible at /manual/en/index.php. This means that
for some reason the 404 handler kicks in when trying to serve that file.
Are you sure there are no URL rewrites or any other magic turned on for
that path? If you use some URL rewrites, see your rewrite log for
details on what is happening.
