in most current versions of PHP4 files can not be created when safe_mode in effect.
neither of (examples follow)
fopen('./dir/this_file_does_not_exist_yet','w');
imagepng($res,'new_image.png');
will work regardless of the permissions.
this can be circumvented by touching the file before it is created:
touch('./dir/this_file_does_or_does_not_exist');
fopen('./dir/this_file_does_or_does_not_exist','w');
