W2K PRO + IIS 5.0 + PHP 4.0.4
5. On "App Mappings" select "Add"
Executable: [Path to PHP]\php.exe (NOTE: NOT A php4isapi.dll!!!)
Was using php4ispai.dll and this wasnt working. Switched to .exe - Runs like a dream.
