Just because I spent a lot of time on it: some mail providers such as hotmail mark php-generated
e-mails as spam and they delete them (or they just put the message in a "Spam folder").
This is because the headers miss a Return-Path. Just add this line
'Return-Path: ' . $sender_address . "\n"
to your header and then everything should work!
