note 68862 added to function.get-loaded-extensions

When you use dl() to load an extension, it won't be listed among the loaded extensions. So if you have a library file that does something like if (!extension_loaded("domxml")) dl((PHP_OS=="WINNT" OR PHP_OS=="WIN32") ? "ext.dll" : "ext.so"); to load the extension only if not loaded yet (ie. if you include the file multiple times) it will always try to load the extension, not just once.
