note 71006 added to install.windows

I made the mistake of setting a 'wildcard application map' for PHP on a Windows 2003 / IIS 6.0 / PHP ISAPI installation. This resulted in "No input file specified" errors whenever I tried to load the default page in my site's directories. I don't know why this broke things, but it did.
